Complete the relevant Application for appointment (COVID-19) form to request Queensland Health appoint an employee as a Contact Tracing Officer, Emergency Officer (General) or Authorised Person under the Public Health Act 2005.

Queensland's response to COVID-19 has required a significant increase in the number of government employees appointed as Contact Tracing Officers, Emergency Officers (General) and Authorised Persons.

These include employees from various state government agencies, such as Queensland Police Service (QPS), the Department of Justice and Attorney-General and the Department of Agriculture and Fisheries. The Queensland Government employee mobilisation workforce initiative has also played a key role.

Application for appointment (COVID-19) form

Use this form to request Queensland Health appoint one or more employees under the Public Health Act 2005 as:

  • a Contact Tracing Officer (CTO)
  • an Emergency Officer (General) (EOG)
  • an Authorised Person.

Do not use this form for applications as part of the:

  • Queensland Government CTO employee mobilisation workforce initiative
  • Queensland Government Public Service Commission (PSC), EOG employee mobilisation workforce initiatives.
