It's now easier and faster to submit your requests to Queensland Shared Services (QSS) with our new online forms.

Queensland Shared Services (QSS) is converting the PDF forms you use to do business with us to fast, paperless online forms.

What PDF forms have we converted?

We've introduced new online forms to replace the following PDF forms:

  • General purpose expenditure voucher (GPEV)
  • Vendor master data
  • Overtime
  • Work hours and work pattern
  • Temporary or casual extension
  • Separation
  • Payslip request.

We're also splitting Corporate purchasing card application and training into two forms.

The Corporate card training application is available now as an online form.

Corporate card application will continue as a PDF (without the training component) and will be available as an online form in March 2021.

What PDF forms will we convert next?

We'll introduce new online forms to replace the following PDF forms:

  • Domestic travel expense
  • Overseas travel expense
  • Purchase order amendment
  • Goods and services requisition
  • Non-payroll employment details
  • Higher duties/relieving at level authority
  • Leave application.

We'll also introduce a new online form for goods receipting and purchase order invoice payments. This will replace the current method of emailing information to QSS.

Why are we converting PDF forms?

We want to make it easier for you to complete transactions with us. Online forms:

  • save you time—no downloading, printing, signing and scanning
  • give you piece of mind—as soon as it's approved, it's with the correct QSS team for processing
  • make it easy—only ask for the information we need to complete the request
  • can be accessed consistently—not restricted by mobile device and web browser compatibility
  • are future proof—don't rely on third-party software for creating and editing
  • help the environment—saving paper and ink.

How are we converting these forms?

QSS is creating the online forms through the PDF Forms Conversion (PDFFC) initiative.

This initiative is adopting an accelerated delivery model, to ensure we convert the most used forms as quickly as possible.

New forms designed with you, for you

Some forms are trickier than others and require more work, and more engagement. As part of our PDF Forms Conversion initiative, we're converting 2 forms that you've told us aren't quite doing what you need them to:

  • Non-payroll person
  • Purchase order request.

We're using human centred design thinking to ensure these forms serve your needs better than the current PDFs. This includes:

  • design inception workshops
  • rapid prototyping
  • showcase workshops
  • user testing.

The feedback we're getting is helping us improve these forms as we convert them. It's also helping us with our edits on all forms.

How can you access the new forms?

Access the online forms by logging in to the QSS Self Service Centre. You can find them in the same location as the PDFs they're replacing.

What else do you need to know?

Once a new form is released, we schedule the removal of the old PDF. We're staggering this process and working with our customer working groups to get the links on your intranets updated. Please remove any PDFs that you've saved to your network or personal drives.
