Note: It is important to record the other driver’s street address (this is located on the back of the driver’s licence).

• Make and model, i.e. Toyota Corolla

Incident details

• Location
• What happened
• Damage to vehicles or property
• Provide images of the incident/property damage if available

Witness details (if available)

Injury

If an injury has occurred, contact the police.
